If you would like to access physiotherapy or any of our other treatments, you will need a referral confirming your diagnosis of a neuromuscular condition and recommending treatment at the Muscular Dystrophy Support Centre.
Referrals can be made by your GP or Consultant. Please ask them to include your contact telephone number so that we can get in touch with you.

Alternatively, a Specialist Neuromuscular Physiotherapist or Neuromuscular Care Advisor can complete our referral form and email it to reception@mdsupportcentre.org.
Please contact us to let us know you have requested a referral. Once we have received it, we will arrange an appointment for your initial assessment.
